Initiating PREP2 in clinical practice requires development of a system to record PREP2 assessments and predictions for clinical staff, and information to give to patients and their families. 

You can initiate Phase 1 while you continue training for Phase 2.

Your staff will need to be familiar with PREP2 documentation so that prediction information can be easily found and understood by all. You will need to introduce your PREP2 documentation during education and training sessions, and ensure it meets the needs of your stakeholders.

Below are templates of documentation for Phase 1 and Phase 2 that you can adapt to meet the needs of your stakeholders and organisation. You will need to consider any approval processes for documentation within your organisation.


Phase 1

  • Prediction handouts for Excellent and Good predictions:

Good Patient Handout (Based on having a SAFE score of 5-7 on day 3 and being over 80 years old)

 

Phase 2

  • Prediction stickers to be placed in the notes for Excellent, Good, Limited and Poor predictions, as well as if further investigation is required or if TMS is contraindicated:
