TMS for PREP2 Homepage

 

This module is for staff who are training to be TMS operators so they can determine MEP status as part of PREP2.

The following modules are all needed if you intend to use TMS for PREP2, and we strongly recommend working through the modules in order. Completing the quiz at the end of each module will provide you with a certificate for that module:

  1. TMS Overview provides background information about TMS and how it works.
  2. TMS Safety Checklist describes the contraindications and safety considerations for TMS and provides a checklist for screening
  3. EMG technique describes how to record from upper limb muscles and troubleshoot potential EMG issues
  4. PREP2 TMS Protocol specifically describes how TMS is used in the PREP2 prediction tool.
  5. Simple MEP identification describes how to detect MEPs in simple scenarios to determine MEP status
  6. Challenging MEP identification describes how to detect MEPs in challenging scenarios to determine MEP status

We recommend that you review our published protocol for using TMS to determine MEP status. The video for this protocol is available below. Access to this video may depend on your institutional access.

Please note since this video was released there has been an update to the TMS technique for PREP2

  • The presence of one MEP minimum is needed to classify someone as MEP+, not 5 MEPs as indicated in the video

You will also need practical training from an expert in TMS in order to develop the necessary skills.

Training will need to involve:

  • Orientation to the TMS equipment and software
  • Training sessions involving people who haven’t experienced a stroke
  • Training sessions involving people with recent stroke who require TMS as part of the PREP2 prediction tool
  • Supervised sessions where you will be supported by a trainer while you develop further experience and skill
  • Competency sessions where your skills will be evaluated by a trainer
  • This process may take several weeks or months, and a template competency document is available so that you can see how you will be evaluated
